As for the NPC window opening twice, are you using a bow? Theres a bug (one that cant be fixed) that makes the client send the 'i clicked on teh npc' packet twice while using a bow.

Failed to synchronize (on stage 1).
Server did not acknowledge.

Last night when I was on it was doing some strange things.  Everyone had lag problems.  Also, when I talked to any NPC the initial screen would pop up twice.  (I would talk to the banker in Wuville, open my bank box, and then get the beginning talk to the banker screen again)
